Specialist Clinical Pharmacist - Renal Medicine
Middlemore
Hospital Pharmacy Department has a vacancy for a full-time Clinical
Pharmacist with experience and up-to-date knowledge in renal pharmacy
to provide clinical pharmacy services within the specialty of
renal medicine.
In this role you will be responsible for the provision of a high
quality medicines management service to inpatients with kidney
disease within a multidisciplinary team environment, including
outpatients and dialysis clinics on a referral and as required
basis. The key priorities for this role will be patient focused
services and ensuring safe and effective supply and use of medicines
in patients with kidney disease. In addition, the role will support
the training and development of pharmacy and other staff in this
area of specialty, as well as departmental activities such as
quality improvement initiatives, medication reconciliation process
and the rational use of medicines.
The successful candidate will have broad clinical pharmacy experience
with at least three to five years recent experience in the renal
and/or adult specialties, and relevant postgraduate clinical qualifications.
You must be registered in or be eligible to obtain one. Good communication
and interpersonal skills as well as effective time management
ability is essential. Ability to motivate and educate staff as
well as a positive attitude is key attributes for this role. The
ability to contribute to quality improvement initiatives within
a team setting is highly desirable.
The renal
service provided by Counties Manukau District Health Board (CMDHB)
is considered to be one of the largest renal services. Supported
by a team of specialist clinicians and nurses providing inpatient
renal medicine and outpatient dialysis services, this position
is an ideal opportunity for a Pharmacist with experience in renal
medicine to develop their career and be part of a team that encourages
and embraces clinical pharmacy input. Both the pharmacy department
and the renal service encourage continuous quality improvement
initiatives. The department and organisation also encourage and
support ongoing professional development through postgraduate

This full-time

Lead Pharmacist, Procurement Full Time, Fixed Term (up to April
2013)
We are looking for a motivated, enthusiastic Pharmacist to work
at the interface between the manufacture / supplier and the clinician
to source specialist medicines not routinely available in NZ and
support colleagues in making cost effective changes to optimise
treatment in their area.
This position provides the opportunity for you to develop your
skills and knowledge as part of the Medicines Management team
and allows you to take responsibility for the clinical and contractual
aspects of pharmaceutical procurement including the development
of clinical directorate reports. As part of this role, you will
liaise with the Formulary pharmacist regarding the introduction
of new medicines, and with the Medication Safety team in producing
warning posters and related information. The role will also involve
participation in drug use evaluations and in clinical ward service
provision as part of a clinical team.
The department's pharmacy computer system enables the department
to produce directorate reports and a working knowledge of Excel
is required. Medicines Management team has a strong IT commitment
and an interest in computerised delivery of information is encouraged.
All ADHB clinical pharmacists now use PDAs as well as the well-developed
MI Intranet site to access medicines information. The post-holder
will work closely with the rest of the Medicines Management team
to guide the progression and development of the service.
The successful applicant will have relevant clinical pharmacy
experience (including a minimum of five years' hospital practice),
the ability to motivate colleagues, as well as showing drive,
enthusiasm and initiative. You must have previous medicines management
experience, be an effective team player and communicator with
excellent interpersonal and time management skills. The post holder
will be committed to raising the quality and profile of the Medicines
Management Team.

Primary Care for a Clinical Facilitator to work in a general practice.
We are seeking an enthusiastic, motivated pharmacist, with excellent
communication skills and relevant clinical/ pharmaceutical knowledge
to develop this exciting role.
This newly established position will provide clinical facilitation
services (with an emphasis on medicines and diagnostic test utilisation)
within a frontline Primary Care settings, by directly supporting
and monitoring GP prescribing and promoting best practice.
The post will endeavour to reduce inappropriate polypharmacy and
diagnostic testing, improve patient health outcomes and deliver
sustainable savings. The role gives you the opportunity to:
- Carry
out individual patient clinical medicine reviews
- Implement
best practice guidelines; " Undertaking Drug Utilisation Evaluation
(DUE) (clinical audit)
- Provide
medicines information to general practice staff
- Analyse
medicines/diagnostic testing utilisation data at all levels.

position is funded and administered by the District Health Board,
operationally you will be General Practice based and work as part
